MINEGOZIIBE ANISHINABE JORDAN’S PRINCIPLE NAVIGATION- CASE MANAGEMENT
1 FTE OFF-RESERVE SUB OFFICE (Winnipeg)
Jordan’s Principle is a legal obligation with a needs-based approach driven by principles of substantive equality, the best interest of the child, and distinct community circumstances. Jordan’s Principle ensures First Nations children can access the products, services, and supports they need, when they need them.
Reporting to the Jordan’s Principle Supervisor, the Navigator will provide support to Minegoziibe members that live off reserve; provide assessment, follow up, and will liaise with other Jordan’s Principle Off Reserve offices as required.
RESPONSIBILITIES
The following list represents the significant duties and responsibilities and is illustrative, not exhaustive.
The goals of case management are first and foremost focused on improving the client’s clinical, functional, emotional, and psychosocial status. Helping clients achieve wellness and autonomy.
Providing case management services; screening, assessing, stratifying risk, planning, implementing, follow-up, and transitioning, communication post transition, andevaluating.
Communicates the service plan(s) to the guardian/family and partnering community organizations for a wraparound approach.
Provides navigation services with a holistic lens, using a trauma-informed and culturally competent approach.
Assess risk for child safety and conduct child development and family functioningassessments.
Overseeing the delivery of services related to family reunification to Indigenous children and their families and communities in Manitoba, with demonstrated success in advancing the needs of children and families.
Work closely with the Jordan’s Principle team as a resource for information, and coordination of care.
Submitting statistical and monthly reports of requests completed.
Other duties as assigned.
EDUCATION, SKILLS AND ABILITIES
Graduation from an accredited college or university with a bachelor’s degree in healthcare/social services/nursing or education.
A minimum of 2 years of related experience.
Comprehensive knowledge of case management procedures
Knowledge of and appreciation for Indigenous ways, teaching and intergenerational impacts are essential.
Ability to thrive in fast-paced environment.
Current Criminal Records/Vulnerable, Child Abuse and Adult Registry Checks
Willing to travel as required/and or requested.
Proficient in Microsoft Office 365
Salary: 60k-90K
